When a blow to the head dims your vision
A neuro ophthalmologist and a trauma doctor explain optic nerve injury
For survivors and families3 min read
Your optic nerve carries what your eye sees to your brain. A hit to the head or face can hurt this nerve, even when the eye has no cut. Your sight may turn dim. Colors may look faded, or a patch of what you see may vanish. [1]
Early on, the eye and the optic disc, where the nerve meets the back of the eye, can look normal. So an eye that looks healthy can’t explain away a big loss of vision. [1]
Get every part of your sight checked right away
Get emergency care for any new vision loss after a crash. The team will test the sight in each eye, your pupils and your color vision. They will also map your visual field, the full area you can see. They look for harm to the eyeball, the retina, the eye socket and the brain. Scans may help find a broken bone or something pressing on the nerve. [1]
Tell your doctors when you first noticed the change. Say so even if other injuries kept you from mentioning it sooner. [1]
What treatment can and can't promise
Treatment depends on what caused the damage. If something is pressing on the nerve, you may need fast action. For some nerve injuries like this, studies haven’t shown that steroids or surgery reliably help sight more than careful watching. Ask your specialist what is known about your case and what risks come with each choice. Be wary of anyone who guarantees a recovery. Regular checkups matter because your vision and exam results can change. [1]
Plan for daily life if your sight stays weak
If your vision stays reduced, ask about reading aids and help getting around. Find out whether it’s safe for you to drive. Go back right away if you lose more sight or a new dark curtain falls across your view. Severe eye pain or new signs of a brain or nerve problem also call for an urgent return. [1]
Questions to bring to your eye doctor
- What part of my eye or visual pathway is hurt?
- Is something pressing on my optic nerve?
- What checkups and vision support should we set up?
Keep notes on what each eye sees
Write down when your vision first changed. Then note what each eye can see on its own. [1]
