Why doctors may keep watching your pregnancy after a crash

An obstetric emergency physician explains the monitoring

You may feel unhurt after a crash while pregnant. Even so, the American College of Obstetricians and Gynecologists (ACOG) advises getting medical care right away. The team first checks you for injuries and makes sure your condition is stable. Then they check on the pregnancy. One worry is bleeding, or the placenta pulling away from the wall of the uterus too early. Those problems may not be easy to see from the outside. [1, 2]

What the monitors are tracking

Later in pregnancy, monitors may track the baby’s heart rate over time. They can also follow contractions of the uterus. How long you’re watched depends on how far along you are and how hard the crash was. Your symptoms and test findings count too. [2]

A normal ultrasound is helpful. It still can’t reliably rule out the placenta pulling away. Before you go home, ask what the monitoring showed. [2]

Speak up about symptoms right away

Tell the team if you have belly pain, a tender uterus, vaginal bleeding or leaking fluid. Also report contractions or a baby who is moving less. These symptoms can change the plan for watching you. Report new symptoms even if earlier tests looked fine. Don’t hold them for your next scheduled prenatal visit. [2]

Go home with a clear plan

Ask if you should call your pregnancy care provider the next day. Also ask when you should come back right away. Confirm your blood type and whether you need more tests or treatment. If you’re sent home, keep the written instructions about when to return. Make sure another adult knows them too. [1, 2]
